Appointments may be made during office hours by calling 410-751-2510. Our office hours are Monday through Friday, 8 a.m. to 4:30 p.m. A charge will incur if appointments are canceled without at least 24 hours notice.

If you are experiencing an emergency, please dial 911 or go to the nearest emergency room. For other after hours concerns call 410-751-2510 to leave a message with the answering service.

New Patient Forms and Information

We ask that you download, complete and return the forms listed below in person, by fax, or by mail at your earliest convenience, but no later than one week prior to your appointment. Please be aware that if we do not receive your records prior to your appointment this may result in a delay or cancellation of your appointment as these records are necessary for proper medical diagnosis and treatment.  Please sign the “Records Release Form” below to give to your referring physician and have them fax any pertinent reports and office notes, lab results, scans and any other test results prior to your appointment.

Please arrive 15 minutes before your scheduled appointment time and be sure to bring your insurance cards, a referral made out to: “Carroll Health Group Endocrinology” (if your insurance requires), photo identification and any applicable co-pays. Checks, cash and most major credit cards are accepted.

Prescription Renewals

Refills will be processed during regular business hours Monday through Friday from 8 a.m. – 4:30 p.m.

Please remember to allow 48 hours for your refill requests and have your pharmacy call us or submit an electronic refill request.

Please note as of 7/1/16, our endocrinologists will only fill or refill endocrine-related medications.


All office visit referrals and co-pays are due at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.
