The Diabetes Program at Carroll Hospital, located in the Tevis Center for Wellness, can help you take on your new responsibilities with confidence. Our experienced, compassionate professionals partner with you to help you learn to manage your diabetes and lead an active, healthy life.

Recognized by the American Diabetes Association, our program offers care, education and support for adults and children in an outpatient setting. Our diabetes services include: 

Diabetes Self-Management Education Programs — we offer both one-on-one diabetes instruction and "living with diabetes" group classes.

Free pre-diabetes and diabetes presentations are also available to church groups, senior centers, health clubs and other private organizations. Please call 410-871-7000 for more information.

How to obtain Diabetes Education Services
1. Ask your physician to complete a referral form for our services. Click for a PDF of the referral form.
2. Fax your referral form to our scheduling department at 410-840-4016.
3. Once we have received your referral, we will contact you to set up an appointment. You may also call our scheduling department at 410-871-7000.
