As the human body ages, it goes through changes that, if not monitored and managed appropriately, can lead to frailty. Specialists at Levindale are trained to take a preventive approach to aging and have developed programs for identifying risk factors for aging-associated diseases. We offer physical, occupational, and speech and language therapies for many conditions. Our therapy team partners with the excellent physicians of LifeBridge Health, providing a multi-disciplinary and holistic approach to patient care.

In addition, Lifebridge Health Rehabilitation Services offers the following components to enhance the flexibility and convenience of our services to patients and the community:

  • Initial evaluations within 48hrs of contact
  • Early morning appointments
  • Partnership with referring physicians to ensure that therapists understand and utilize the physician's approach to patient care within their treatment plans
Preparing for Your First Visit

Here are a few reminders to help ensure that your first appointment runs as smoothly as possible:

  • Bring your insurance cards and photo ID
  • Complete your paperwork upon arrival (consent form, notice of privacy, medicare as secondary payer (as applicable))
  • Remember your doctor’s prescription
  • Wear loose, comfortable clothing, snickers
House Rules

In order to ensure that you maximize your therapy visits, here are some suggestions:

  • Plan to arrive 10- 15 minutes prior to your appointment
  • Call at least 24 hours ahead if you want to cancel or re-schedule appointment
  • Make sure to double check your next appointment before you leave
