Welcome to My LifeBridge Health

With the My LifeBridge Health Patient Portal, your complete health information is available 24 hours a day, seven days a week via your computer, tablet or smartphone. It’s safe, secure and super convenient—and all in one place.

With the My LifeBridge Health Patient Portal, you can:

  • View your vital signs
  • Request or direct schedule an appointment with participating providers online
  • Request a prescription refill
  • Get lab and radiology results within 36 hours (pathology results are also available)
  • Send secure messages to your doctor
  • Fill out forms online prior to your visit
  • Check your medical history
  • Download discharge instructions
  • See your current medications, allergies, immunizations and procedures
  • Connect fitness and dietary trackers to your health record
You can access your personal health information through the Apple Health icon on your iPhone. 

We have made it easy for patients to review their test results through our online LifeBridge Patient Portal delivering results of these tests as quickly as possible (six days after the test was completed). If you are already enrolled in Patient Portal, you will receive an email alert that new lab test results are available. Log into the Portal to receive your results.

How to Get Started

Fill out the My LifeBridge Health online request form, which can be downloaded here. You will receive an email with a link and instructions to complete your registration.

If you are experiencing trouble logging into your My LifeBridge Health account or experiencing other technical issues, please call 410-601-WELL.

My LifeBridge Health is intended for routine health management. If you are experiencing a medical emergency, please call 911.

API Request

We offer the ability for you to securely connect some of the health management apps you may use (i.e. fitness trackers, dietary trackers, etc.) to your health record through your patient portal account. This allows for easy access to health and wellness data, enabling you and your care team to make informed choices. If you are interested in this option, fill out the API form, which can be downloaded here.

Once we receive your request, we will work with the appropriate vendors to determine if they meet the technical requirements in order to establish a secure connection.
